Health Insurance Trend Information The purpose of this activity is to input data from the health insurance provider’s quarterly report for utilization by contract and provide summary information for Executive management and the Employee Benefit Suggestion Committee. 1. Obtain quarterly utilization report from CareFirst. 2. Retrieve Excel spreadsheet Health/CFTrend {YYYY}. 3. Enter quarterly data in following ranges by plan type: a. For claims and premiums, insert row above last quarter of page; b. Copy last line into space; c. Enter current quarter end date and data by line of coverage; d. Enter total as proof; e. Revise 12 month rolling totals; f. Repeat for each group 4. Summary ranges within worksheets for claims and premiums will require the following: a. Insert row above last quarter of page; b. Copy last line into space; c. Enter current quarter date and total as a proof; d. Revise 12 month rolling totals as needed 5. For the worksheets, Ratio (Utilization) ranges, (under 65) without dental, Difference; Allocation of Claims and Allocation of Premiums: a. Insert row above last quarter of page; b. Copy last line into space; c. Enter current quarter date; d. Repeat for each range as needed 6. Print summary of schedules as needed for presentation. a. Utilization ranges for Benefits Committee; b. Full Schedule for Comptroller review and file copy G. Form 5500 - Pretax Medical and Prescription Card Program The purpose of this activity is to prepare Form 5500 for health benefits' participants who pay pre- tax per the WCBOE Sect. 125 Plan. Form 5500 is due by March 31 of the following year. 1. Obtain Health Care counts by coverage from MIS used for final projection as of 6/30. a. Total eligible employees is the total of the report b. Health code "W" is those employees who waived benefits or "opted out". c. Health code "D" are spousal employees. 2. Obtain copy of COBRA Participants from Human Resources. 3. Retrieve Current Earnings Register as of 6/30 from Payroll Dept. and copy last page showing employees paid (includes substitutes). 4. Calculate Participants a.
