School social worker generally/usually identifies school and community resources. School social worker demonstrates inconsistent knowledge or familiarity with school and community resources. School social worker consistently and reliably identifies school and community resources. School social worker demonstrates thorough knowledge of resources available through the school and community, identifying potential partnerships and sharing findings with school personnel, students and families. School Social Worker always identifies school and community resources, understands how families qualify for services. School social worker also takes the lead in developing resources not met in the community.
